Apprenticeship Forms

The documents you need from the start to finish on an Apprenticeship.

All the Support You Need

Certificate Request Forms

Submit a claim for an Apprenticeship certificate. But remember there is a separate certificate request form for each industry and level. We also require within each application a permission slip signed by the learner, giving authorisation for the training provider to apply for the certificate on their behalf.

Employee Rights and Responsibilities Workbooks (ERR)

We have designed workbooks to help you when starting an Apprenticeship. They help you understand issues such as your contract of employment, legislation related to your work such as health and safety and employment legislation that you and your employer should follow.

Additional Employer Requirements

If you have a new learner who wants to complete an additional employer requirement that is not listed within the framework, please contact our Apprenticeship Certificate Administrator for confirmation that it will be accepted as a substitute. Please do not wait until the learner has completed the Apprenticeship. This will be too late to correct if we disagree and will disadvantage the learner.Contact our Apprenticeship Certificate Administrator on 02476 858 431.

Specification for Apprenticeship Standards in England (SASE) and Wales (SASW)

The SASE and SASW is the criteria that all Apprenticeship frameworks have to meet so they can attract government funding. All Apprenticeship frameworks will need to comply with SASE/SASW by 6th April 2011. We are working on the changes that need to be made and in most cases, there will only be minor tweaks for all frameworks.

Personal Learning and Thinking Skills (England only)

Awarding Organisations have designed records of achievement to help you throughout your Apprenticeship. They help you understand Personal Learning and Thinking Skills (PLTS) such as team working and self management.
